Hospice Team

Our specially trained hospice team includes:

Levels of Care

Routine hospice care includes visits to provide assessments, skilled care, personal care, and support as well as coordination of medications, medical equipment and supplies.

Special circumstances may require a higher level of hospice care for a short period of time in which we provide:

  • Respite care to provide relief for the primary caregiver

  • Acute inpatient care when pain and symptoms become challenging to control

  • Continuous Crisis Care around the clock to manage complex pain and symptoms

Where is Care Provided?

St. Joseph Villa Hospice brings their services to wherever the patient considers home, which may be a private residence, a senior or assisted living community or a skilled nursing center. Regardless of the setting, our hospice team coordinates care with the patient, family and all healthcare providers and brings physical, emotional, and spiritual care and support to the family.
