St Joseph Villa Homecare offers personalized patient-centered home health services to patients in the comfort of wherever they consider home. As one of the first homecare agencies in Omaha, St Joseph Villa caters to the individual needs of our patients, ensuring that their physical, emotional and mental needs are met. Our goal is to help you live safely and independently at home.

Our experienced team of professionals are highly trained to provide medical assistance for various medical conditions while educating and empowering you to manage your health through tools and resources. Whether you are recovering from an injury, illness, surgery, hospital stay or managing other ongoing medical conditions, such as CHF, COPD or Diabetes, we are here for you with love, care and understanding.

Rehabilitation

Home care rehabilitation allows patients to regain ability and develop independence with their daily activities in the comfort of their own home. Our staff of expert physical, occupational and speech therapists work together with the home care team to restore patients to a healthy physical and phycological state.

Skilled Nursing

St. Joseph Villa offers skilled nursing services for patients who require medical attention. A nurse is assigned as case manager, who coordinates the plan of care prescribed by your doctor to meet your goals. Our licensed and trained nurses will oversee and provide services that may include, but are not limited to:

  • Multi-system assessments, including vital signs and blood pressure monitoring  

  • Sets up a personalized plan of care with the patient’s goals in mind

  • Educates patients, families and caregivers about patients’ illnesses and treatment

  • Monitors patient’s symptoms and health and addresses early symptoms that could lead to hospitalization

  • Assists with medication reconciliation, management and supervision

  • Injection medication education and management

  • Provides pain, symptom and disease management

  • Provides wound care, ostomy care, in-home lab draws, bladder and bowel training, self-care of chronic conditions and other medical services

Physical Therapy

Following orthopedic surgery, illness, onset of neurological conditions, generalized weakness and other conditions affecting mobility, our licensed physical therapists help patients achieve their maximum level of functioning by developing an individualized program to improve each patient’s mobility. Our physical therapy services include, but are not limited to:

  • Gait, transfer and mobility training

  • Therapeutic exercises  

  • Range of motion, dexterity, and muscle strength training

  • Home exercise training

  • Prosthetic training

  • Self-care and ambulation techniques

  • Durable medical equipment and assistive device training, such as the use of crutches, canes, walkers or wheelchairs

Occupational Therapy

Loss of function and ability to perform everyday activities of daily living including dressing, bathing, using the bathroom and meal preparation are all addressed by our registered occupational therapists. The occupational therapist helps patients regain the ability to perform their everyday activities to help them live safely and independently at home. Our occupational therapy services include, but are not limited to:

  • Activities of daily living training

  • Improve functional abilities

  • Fabricate and apply splints

  • Assist in selection and use of adaptive equipment

  • Strength and coordination enhancement

  • Energy conservation techniques

  • Home modification recommendations to reduce fall risk and improve safety

Speech Therapy

Neurological, orthopedic, cognitive and traumatic conditions that affect the ability to speak, swallow, problem solve, word find and other related communication difficulties are addressed by a licensed speech pathologist. Our specialized speech therapists assist patients in achieving his or her maximum potential relating to communication and oral-motor skills. Our speech therapy services include, but are not limited to:

  • Evaluate and treat language deficits resulting from disease or stroke

  • Evaluate and treat swallowing disorders

  • Evaluate and treat motor speech deficits

  • Evaluate and treat voice disorders

  • Counsel and teach families strategies related to specific diagnosis and develop verbal and nonverbal ways to improve communication

Home Health Aide/Private Duty

During your rehab, home health aides can be provided for assistance with bathing and other care needs related to activities of daily living and personal care. Our home health aide services include, but are not limited to:

  • Assistance with personal and oral hygiene

  • Assistance with daily tasks such as bathing, dressing, eating and meal preparation

  • Medication reminders and pharmacy pick up

  • Housekeeping, laundry, shopping and errands

  • Provide companionship and support

  • Skin care

  • Respite care

  • Care from one hour up to 24 hours per day

Medical Social Workers

As you navigate the intricacies that are sometimes involved in managing your injury or illness, our Medical Social Workers are here to provide helpful information, resources and assistance. The Medical Social Worker help to break down barriers of any social, environmental, emotional, financial and psychological factors that may hinder your progress. Our Medical Social Worker services include, but are not limited to:

  • Adjustment to physical illness

  • Advocacy and community resources for patients and families

  • Provides short-term counseling and crisis intervention

  • Provides community support services

  • Provides resources for community services, entitlement programs and public benefits

  • Assists with long-term care planning

  • Assists in accessing financial assistance

Additional Services

Wound Care

Our skilled wound care specialists possess the knowledge and experience to prevent wound complications and promote wound healing. We treat wounds, as well as, the underlying disease process and cause of wounds.

Medication Management

Medication management and education is an ongoing part of each patients plan of care. We coordinate, monitor, and evaluate medications throughout the course of care to ensure patient safety and compliance with a specific medication regimen.

Disease Management

Our home health team provides education, support and tools to patients with chronic illnesses to prevent ongoing ER visits and rehospitalizations. Some of the chronic illnesses include but are not limited to, COPD, Stroke/CVA, Diabetes, CHF, Ostomy, CAD (Heart Disease), Respiratory Illness, Enteral Nutrition and Hypertension.
