St. Joseph Villa offers personalized patient-centered home health services to patients in the comfort of wherever they consider to be home. As one of the first homecare agencies in Omaha, St. Joseph Villa caters to the individual needs of our patients, ensuring that their physical, emotional and mental needs are met.

 

Our goal is to help you stay safe at home and live as independently as possible. Our experienced team of professionals are highly trained to provide medical assistance to various medical conditions while helping to empower you to manage your health through tools, resources and education. Whether you are recovering from injury, illness, surgery or managing other ongoing medical conditions, we are here to help.

What is homecare?

Home health care is physician-directed, short-term care aimed to help patients avoid or recover from an injury, illness, surgery, hospital stay or manage other medical conditions. Care must be prescribed by a physician and the homecare team follows the plan of care established with you and your physician. Homecare is provided through visits from an experienced, specialized team of clinicians on a short-term basis until your individual goals are met. Our highly skilled team of nurses, physical therapists, occupational therapists, speech therapists, social workers and home health aides bring compassionate personalized care to you and your family.

Am I eligible?

To qualify for Home Health Care, the following criteria is required:

  • Patient must:

    • be confined to the home (homebound)

    • need the help of medical equipment such as wheelchairs, walkers, canes and/or crutches

    • require the use of special transportation

    • need the help of another person to leave their place of resident due to an illness or injury

    • exhibit that leaving home is a significant and taxing effort

    • Patient needs skilled services that are reasonable and medically necessary, ie:

      • Skilled Nursing, Physical Therapy, Occupational Therapy or Speech Therapy

    • Patient must currently be under the care of a Physician

    • Patient must requires services provided through a home health plan of care that is reevaluated by their physician periodically

    • Patient must have a face-to-face visit with their physician within no more than 90 days before or 30 days after the start of home care services

Please note: Patients may leave the home and still be considered homebound under certain circumstances: to receive health and medical treatment, attend religious services, attend adult daycare programs and other infrequent events such as graduations, funerals and the beauty parlor

What does it cost?

Home health care is a benefit that is 100% covered by Medicare for eligible patients. We also work with Medicare Advantage, Medicaid, Third party insurances, Veterans Administration and private pay. St. Joseph Villa works closely with patients and insurance providers to determine potential out of pocket costs, if any.
